Liability Waiver & Release

LIABILITY WAIVER & RELEASE

Superbase JC / Basement Sports Inc. — Applies to all camps, classes, parties, events, and activities.

1. Assumption of Risk

I understand that participation in sports, physical activities, camps, classes, parties, and events at Superbase JC involves inherent risks, including but not limited to physical injury, illness, and in rare cases serious injury. I knowingly and voluntarily accept and assume all such risks, both known and unknown, on behalf of myself and my child(ren) named below.

2. Release & Waiver of Claims

In consideration of being permitted to participate, I hereby release, waive, and discharge Superbase JC (operated by Basement Sports Inc.), its owners, officers, employees, coaches, volunteers, agents, and affiliates from any and all liability, claims, demands, or causes of action arising out of or related to any loss, damage, or injury that may be sustained during participation, to the fullest extent permitted by law.

3. Medical Authorization

I authorize Superbase JC staff to secure emergency medical treatment for my child(ren) if needed, and I accept financial responsibility for any such treatment. I confirm that my child(ren) are physically able to participate in the activities offered.

4. Indemnification

I agree to indemnify and hold harmless Superbase JC and Basement Sports Inc. from any claims, actions, or damages, including reasonable attorneys' fees, arising from my or my child(ren)'s participation.

5. Rules & Conduct

I agree that my child(ren) and I will follow all posted rules, safety guidelines, and staff instructions. Superbase JC reserves the right to remove any participant whose conduct is deemed unsafe or disruptive.

6. Binding Agreement

This waiver is binding upon me, my child(ren), and our respective heirs, executors, and assigns. I have read this Liability Waiver & Release in its entirety, understand its terms, and sign voluntarily.
